You will need to create one at least for VMware in order to set the
DATASTORE_LOCATION in the cluster so the datastores can inherit it. In
any case, it would be recommended to have one cluster per hypervisor.

Yes, the template used to create the cluster must include the
DATASTORE_LOCATION, although this can be updated later. In any case,
the DATASTORE_LOCATION _must_ be present before the datastore is
created.

In the ESX, only the system and one image datastore need to be
register (and potentially, more than one image datastore). In
OpenNebula 4.2 they cannot be 0 and 1, since they are created on
OpneNebula first boot and they won't have the proper
DATASTORE_LOCATION inherit.

To check that all is good, the BASE_PATH of both the system and image
datstore for VMware should contain "/vmfs/volumes"
