CONTEXT parameters can be used from inside the VM for initializing the image during startup.

No. The $NAME refers to the name of the initialized VM. In your example this is "ttylinux". 
You can define whatever you want, e.g. based on the $VMID parameter.

What do you mean with "should"?

Typically, it would be the user that initialized the VM. --> $USER[NAME]
